Please include copies of a current MVR (links to the Virginia, North Carolina, & Maryland DMV web sites are provided below), medical card, and medical card long form. DOT requires a complete history of driving jobs for the last ten years.

• Minimum age requirement: 21 years of age
• Social Security Number
• Current driver license number and driver license history
• Employment history for the last ten years
• History of all traffic convictions and accidents for the last five years, including month/year and complete details. (Convictions of reckless driving, suspensions, excessive speeding, etc. will affect employability.)
• All Criminal history
• History of any DUI conviction(s) (Convictions within the last three years will prevent employment.)
• MVR

• Virginia DMV
• North Carolina DMV
• Maryland DMV
